§ 19-17. Application.

  • (a)

    Applicants for a permit under this article shall file with the city clerk a sworn application in writing on a form to be furnished by the city clerk. The application shall require the following information:

    (1)

    The name, address or headquarters of the person applying for the permit and the local address of the applicant if any.

    (2)

    If the applicant is not an individual, the names and addresses of the applicant's principal officers and managers.

    (3)

    A brief description of the nature of the business and the goods to be sold and if the applicant will be involved in solicitations, the purpose for which such solicitation is to be made, and the use or disposition to be made of any receipts therefrom.

    (4)

    An outline of the method or methods to be used in conducting the sales or solicitations.

    (5)

    The period during which the sales or solicitations shall occur.

    (6)

    In the event solicitation is involved, a full statement of the character and extent of the charitable work being done by the applicant within the city.

    (7)

    A statement as to whether or not the applicant has been convicted of any crime, misdemeanor, or violation of any municipal ordinance other than a traffic violation, the nature of the offense and the punishment or penalty assessed therefor.

    (8)

    The last municipalities, not to exceed three (3), where the applicant carried on business or solicitations immediately preceding the date of application and the addresses from which such business or solicitations were conducted in those municipalities.

    (b)

    At the time of filing the application, a fee as determined by resolution of the council shall be paid to the city clerk to cover the cost of investigation of the facts stated therein.
